Health Savings Accounts (HSAs)

Bring your HSA credit card. No card? We also accept checks from your account.

Care Credit

  1. Apply for Care Credit ahead of time. Visit their website to apply.
  2. Once approved, bring your reference number or a copy of the Care Credit Card to your appointment. Without it, we’ll need to reschedule.

Frequently Asked Questions

  • Please arrive 15 minutes before your appointment to complete any necessary paperwork and ask any questions you may have. We want to ensure you have ample time to address all your concerns.
  • Wear loose-fitting clothing or shorts if you have a lower extremity complaint. If you are female and have shoulder pain, we advise wearing a sports bra for your comfort.
  • Bring a copy on CD of any imaging done in the past 2 years related to your condition, including X-rays or MRIs.

Important:

  • If you have X-rays, they must be a CD copy, not just a report.
  • Feel free to bring any reports, but they are not a substitute for the images on CD.
  • Even if the imaging center says they will send the images, please request a CD copy and bring it with you. They often send only the reports.
